Adelaide Therapy Hub is an NDIS registered specialist provider offering support coordination and allied health therapy services to NDIS participants. We have been providing support coordination and allied health services since 2018.
We are currently offering NDIS Support Coordination Services (Levels 1 , 2 & 3) and Psycho-Social Recovery Coaching.
Adelaide Therapy Hub is taking referrals in Adelaide Metro, Adelaide Hills and the Limestone Coast areas.

How can we help you?
Adelaide Therapy Hub is here to support you in navigating the NDIS and accessing your NDIS plan to its fullest potential. Our support coordinators will work with you to gain access to services, including mainstream, informal and community supports, through your NDIS plan.
Our support coordinators will work closely with you to help access all the supports available to you, in line with your goals and also your NDIS budget. This ranges from making sure you understand your NDIS plan, setting up service agreements with providers, resolving any issues or complications with selected services, building community networks, managing budgets and aiding you to develop the skills that will help you to continue to see the best results from your NDIS plan.
We want to create a pathway for you to have complete confidence in your NDIS plan, with seamless access to the services and supports you need to achieve your goals.

Supporting you.
Our support coordinators will work with you and your support network, to explore your options and develop confidence in accessing supports to make the most of the opportunities within your NDIS plan.
Once services are established, your support coordinator will continue to provide support by overseeing services to ensure they are cost effective, meeting your needs, and progressing towards your goals. If there are any challenges or obstacles along the way, your support coordinator will assist to manage these with the aim of eliminating any disruption to your already established and on-going services.

Understanding a support coordinator's role.
Our aim is to help you:
Understand and use your NDIS funding to achieve your goals
Connect you with NDIS providers, community, mainstream and other government services
Build your confidence and skills to use and coordinate your supports
Over-see on-going expenditure and maintain close supervising of budgets within the plan
Support you in all NDIS meetings including plan reviews. In some circumstances, you may need more funds to achieve certain goals, your support coordinator will give evidence to the NDIA to request additional funding
Liaise with all providers to ensure continuity of supports during plan renewals/plan reviews
Communicate with you regularly to ensure that all services are meeting your needs and expectations
Report to the NDIS on outcomes and progression of services and support in current plans.
Check-in regularly to see how your day is and make sure you are happy and safe through every process.

Am I eligible for support coordination?
When creating your NDIS plan, the NDIS planner will make a decision whether support coordination is reasonable and necessary for your on-going support. The decision is made based on your NDIS goals and your plan objectives. This will then be funded as part of your NDIS package. Specialist Support Coordination (Level 3) can be assigned to you if you have complex needs.
Support Coordination funding is part of the Capacity Building Supports section of an NDIS plan and it will be stated at which level you have been funded for.
If you do not have this in your current plan and are needing assistance with connection to supports, we recommend contacting the NDIA to request that this be reviewed.
There are three different levels of Support Coordination:
Level 1 - Support Connection is short-term assistance to implement your NDIS plan, including identifying service providers and negotiating Service Agreements.
Level 2 - Support Coordination is a longer-term support to increase your capacity to maintain relationships, manage service delivery tasks, live more independently and be included in your community.
Level 3 - Specialist Support Coordination is a specialised coordination that aims to manage challenges in your support environment and ensures consistent delivery of service.
